The Home Exercise Program Library gives patients one organized place to record the exercises a physical or occupational therapist, physician, or other provider has prescribed. Readers write in each exercise, how often to do it, and any instructions, then use a weekly log to check off completed sessions and note questions or concerns to share at the next visit. It does not prescribe exercises; it organizes the program the reader's own provider has given. It is also part of the CarePlanCompass™ Guide Provider-Directed Exercise Resources. It is educational and organizational support and does not replace individualized medical, rehabilitation, or provider guidance.
