The Phase-Based Recovery Roadmap gives patients and families a clear picture of how recovery usually moves forward after surgery or injury. It presents four general phases (protection, controlled movement, functional rebuilding, and return to activity), what each phase commonly focuses on, and space to record where the care team says the reader is today. The roadmap uses no fixed dates, because progress depends on the individual plan of care. It pairs well with any CarePlanCompass™ surgical Companion. It is also part of the CarePlanCompass™ Guide Recovery Phases & Returning to Activities. It is educational and organizational support and does not replace individualized medical, rehabilitation, or provider guidance.
