More details

Daily Care & Routines is a CarePlanCompass™ Guide that helps you turn a care plan into a daily routine that fits your real life. It moves through three parts: Make Your Care Plan Fit Your Day, which helps you choose today's most important need, understand why routines help, work with your energy, see how your care fits into each area of everyday life (taking care of yourself, running your home and your life, managing your health, rest and sleep, work and school, play and free time, and relationships and community), start one small habit, and build your routine in layers; Keep Your Routine Going, which covers setting up your home, supplies, reminders, a medicine routine, solving problems when the routine breaks, hard days and big changes, asking for help, and talking with your care team about your routine; and Your Routine Pages, with a daily support check, an everyday life check-in, a routine planner, a small habit plan, supplies and refills, a hard-day plan, a habit tracker, a weekly check-in, helpers, and notes. References are listed at the end, and the Guide pairs well with the Lifestyle & Functional Health Companion. It is educational and organizational support and does not replace individualized medical, rehabilitation, or emergency care.
